A practice does not lease square footage. It leases plumbing, power, floor loading, parking ratio and patient access. Most of the buildings in a search cannot deliver all five.

Why practices keep choosing the Tysons corridor

Tysons and McLean function as one submarket, and it now holds more than 220 healthcare and dental locations. Physician offices, dentistry, mental health and therapy practices, optometry and diagnostic imaging, alongside Kaiser Permanente’s Tysons Corner Medical Center on Westpark Drive and recent commitments from systems including MedStar Health.

Concentration matters more in healthcare than in almost any other product type. Referral relationships are geographic. Patients recognise a medical destination. And four Silver Line stations, McLean, Tysons, Greensboro and Spring Hill, put most of the corridor within half a mile of rail, which widens both your patient catchment and your hiring pool.

The five things that actually disqualify a building

Rate is rarely what kills a medical requirement. These are.

  • Plumbing. Where the stacks run decides where your operatories, sinks and sterilisation bay can go. On an upper floor in a 1980s Tysons tower, that answer is often "not where you want them".
  • Power and HVAC. Imaging, sterilisers and compressors need dedicated circuits and sometimes after-hours conditioning. Base building capacity varies enormously across the corridor.
  • Floor loading and ceiling height. The constraint for imaging, and the reason ground floor and purpose-built medical buildings command what they do.
  • Parking ratio. Tysons has roughly 25,000 structured spaces, but a general office ratio tested against real patient volume plus staff frequently fails. This is a clinical operations issue, not a real estate one.
  • Access. Ground floor entry, elevator wait times, wayfinding and how an elderly or mobility-limited patient actually reaches your door.

Screening for these before touring is most of the value. It is also why a general office broker and a healthcare requirement are a poor match.

Second generation space against a shell

2026 construction guides put dental and medical build-out at roughly $150 to $350 a square foot for a renovation, and $200 to $450 and above for new construction, before equipment. A four-operatory practice should expect to budget separately for chairs, delivery systems and cabinetry.

Which means a second generation medical or dental suite with usable plumbing can be worth materially more than a lower face rate on a shell, and a large improvement allowance on the wrong building can still leave you writing a cheque. Every option gets modelled the same way: rate, term, escalations, allowance and real build-out cost, over the full term.

Where the leverage sits right now

General office in Northern Virginia favours tenants. Medical does not. National medical office occupancy reached record levels in 2026 with absorption up sharply year over year, while new construction ran at around two percent of inventory. Landlords who control a building that can take a clinical fit-out know exactly how few alternatives you have.

Time is the lever you control. Eighteen to twenty four months of runway lets you screen the corridor, find the two or three buildings that genuinely work, and create competition between them. Six months does not, and the pricing reflects that.

The spread within that range is wide: a second generation suite in a purpose-built medical building and a conversion in a commodity tower are not the same product, and the improvement allowance frequently moves more money than the rate does.

How long does it take to open a practice in new space?

Plan on twelve to twenty four months from starting the search to seeing patients. Condition drives the timeline more than size: a second generation suite with usable plumbing can move quickly, while a cold shell adds design, permitting and construction. Fairfax County permitting and inspections for clinical use should be built into the schedule rather than discovered during it.

Is second generation medical space always the better option?

Usually, but not automatically. Existing plumbing, a sterilisation bay and a working reception can save six figures and months of schedule. What matters is whether the existing layout suits how your practice actually runs. Inheriting someone else’s workflow at a discount is not a saving.

Which buildings in Tysons can take a clinical fit-out?

Fewer than the search results suggest. The corridor mixes purpose-built medical buildings, ground floor positions in mixed-use developments, and 1980s and 1990s office towers with deep floorplates that were never designed for clinical use. Screening on plumbing risers, electrical capacity, floor loading and parking ratio typically removes most of a long list before anyone books a tour.

Who pays for tenant representation?

In most Northern Virginia transactions the commission is paid by the landlord and is already built into the economics of the building, whether or not you bring your own advisor. Fee arrangements are agreed in writing before any work starts, so there are no surprises either way.
